Eligibility Information 

Parents/Athletes:  Please follow the directions carefully to fulfill eligibility requirements for participation in Olympic High School Athletic Programs

Step 1    Open and Read the Athletic Code and Insurance documents for the sports you plan to participate in.

Step 2     If you don’t have a valid physical (valid for 24 months) please print the Student Physical Form and schedule an examination. Please contact the Athletic Director for the expiration date for the last physical examination. A valid physical must be on file for participation.

Step 3     Print and complete the Medical History/Emergency Card

Step 4     Print, complete and sign the Concussion Information Acknowledgment Document

Step 5     Print, complete, initial and sign all information on the Athletic Clearance Form

Step 6     Return the completed Athletic Clearance Form, Medical History/Emergency Card, Concussion Information Acknowledgment Document and Student Physical Form (if one is not on file) to the Athletics Office.

Note: To be eligible for athletic participation, OHS Athletes are required to purchase an OHS ASB ($45) card and pay the CKSD Participation Fee ($75). Athletic Fees can not be paid until after tryouts for cut sports.

Athletic Clearance Form (completed form required)

Concussion Acknowledgment (completed form required)

Student Physical Form (valid physical must be on file - expiration after 24 months)

Emergency Card (completed form required)

Insurance Forms by Sport (required reading)    

OTHER INFORMATION

ACADEMICS:

Students must pass five (5) classes in the semester prior to participation to be fully eligible to participate and compete. Grade checks are conducted prior to and during the seasons.
